We put the littlest cowfish back into the main tank. We did a full treatment of erythromycin (4 days) then kept him in quarantine another day and a half, feeding him flake and small pellet food, both of which he seemed to like very much (at least much better than the shrimp pellets).
Once back in the tank, he found the other cowfishes and they seemed to get along all right. I'm still concerned about his missing pectoral fin, but there's nothing we can really do about that.
